The Health Information Information Form is a basic intake information that provides the necessary information to get started with your first visit.

Physician's Permission form
If you are under a doctor's care, it is strongly advised that you receive a written recommendation for massage or bodywork prior to any session. Depending on the condition, approval from your doctor may be required. Your provider may use the Permission Form below, or provide his/her own form.

Physician's Referral form
There is growing support for the use of massage and Asian bodywork for pain and other symptoms. Your medical provider may wish to refer you for massage.
